The dog license, issued pursuant to this title, shall set forth a description of the dog licensed, the year in which issued, the date of rabies vaccination, the veterinarian's certificate number or the serum number, the sex of the dog, the number of the license, and a statement showing the amount of the fee and that the same has been paid, and the name and address of the owner.
